考虑患者心理效用的应急手术调度研究

摘要: 为解决突发医疗事件背景下应急手术调度的患者满意度及手术效率低下的问题,将应急手术调度问题看作柔性流水车间调度问题,深入考虑等待时间和主治医生分配结果对患者满意度的影响,基于心理账户理论,建立患者心理效用函数,构建患者心理效用最大和手术完成时间最短的双目标手术调度模型。鲸鱼优化算法常用于求解各类优化问题,但算法的全局搜索能力和收敛速度有待提升,为此做出以下改进:(1)采用精英反向学习策略提高初始种群适应度;(2)离散化并提出两种交叉方式以有效解所提模型;(3)采用随机差分变异策略对种群位置扰动以防止陷入局部最优。使用改进的鲸鱼优化算法对应急手术模型求解,最后通过算例实验表明:改进后的算法性能有效提高,求解结果更优。

Abstract: In order to solve the problem of low patient satisfaction and surgical efficiency of emergency surgical scheduling in the context of medical emergencies, the emergency surgical scheduling problem is regarded as a flexible assembly workshop scheduling problem, and the influence of waiting time and attending physician allocation results on patient satisfaction is deeply considered, and the patient mental utility function is established based on the psychological account theory, and a dual-objective surgical scheduling model with the greatest patient psychological utility and the shortest operation completion time is constructed. Whale optimization algorithm is often used to solve various optimization problems, but the global search ability and convergence speed of the algorithm need to be improved, so the following improvements are made: (1) the elite reverse learning strategy is used to improve the initial population fitness, (2) the discretization and two crossover methods are proposed to effectively solve the proposed model, and (3) the stochastic difference variation strategy is used to disturb the population position to prevent it from falling into the local optimum. The improved whale optimization algorithm is used to solve the emergency surgery model, and finally the example experiments show that the performance of the improved algorithm is effectively improved, and the solution results are better.
